Thank you for choosing me to complete your home study, I look forward to working with you. Listed below is the information and items that I will need to complete the study. Typically, your study will be completed and submitted to the court within 4-6 weeks. Payment is due at the time of the initial interview that will take approximately 2-3 hours.

• Name, address, phone, and email address for your attorney.

​

• Name of court and court case number.

​

• 5 personal reference letters on your behalf. Three should be from family members and two from friends. Each letter needs to include their name, relationship to you, how long they have known you, a description of your character and their recommendation, from friends and family that support you in this process. They can email their letters directly to me at the above email address putting your name on the subject line.

• A completed criminal background check. This can be done through the Texas Department of Public Safety or IdentoGO at www.identogo.com. You need to send the results to me so that I can include them in the report and also have your attorney file them with the court clerk.

• A completed CPS background records check through the Texas Department of Family and Protective Services. This can be done by going to the website: www.dfps.state.tx.us. On the home page, on the bottom right hand side click on background check under "find services" section. Then click on the tab "Form 2970 Request for Child Abuse/Neglect Central Registry Check". Download, complete and submit the notarized form to DFPS for processing the request. You need to send me the results so that I can include them in the report.
